Dayasagar Parajuli <\/strong>4<sup>th\u00a0 <\/sup>April 2020, <\/p>\n\n\n\n<p>Corona virus disease 2019 (COVID -19) is an <a href=\"https:\/\/en.wikipedia.org\/wiki\/Infectious_disease\">infectious disease<\/a>\u00a0caused by\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Severe_acute_respiratory_syndrome_coronavirus_2\">severe acute respiratory syndrome corona virus 2<\/a>\u00a0(SARS-CoV-2).\u00a0The disease was first identified in December 2019 in\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Wuhan\">Wuhan<\/a>, the capital of China&#8217;s\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hubei\">Hubei<\/a>\u00a0province, and has since spread globally, resulting in the ongoing\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/2019%E2%80%9320_coronavirus_pandemic\">2019\u201320 corona virus pandemic<\/a>.\u00a0Common\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Symptom\">symptoms<\/a>\u00a0include\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fever\">fever<\/a>,\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Cough\">cough<\/a>, and\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Shortness_of_breath\">shortness of breath<\/a>.\u00a0Other symptoms may include\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Myalgia\">muscle pain<\/a>,\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Sputum\">sputum<\/a>\u00a0production,\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Diarrhea\">diarrhea<\/a>,\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Sore_throat\">sore throat<\/a>,\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Loss_of_smell\">loss of smell<\/a>, and abdominal pain.\u00a0While the majority of cases result in mild symptoms, some progress to viral\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pneumonia\">pneumonia<\/a>\u00a0and\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Multi-organ_failure\">multi-organ failure<\/a>.\u00a0As of 3 April 2020, more than 1,010,000\u00a0cases of COVID-19 have been reported in more than two hundred countries and territories,\u00a0resulting in over 53,000 deaths.\u00a0More than 211,000 people have recovered. <\/p>\n\n\n\n<p>The virus is mainly&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Transmission_(medicine)\">spread<\/a>&nbsp;during close\ncontact,&nbsp;and by&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Respiratory_droplets\">small\ndroplets<\/a>&nbsp;produced\nwhen people cough, sneeze, or talk.&nbsp;Respiratory droplets may be produced\nduring breathing but the virus is not generally&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Airborne_disease\">airborne<\/a>.&nbsp;People may\nalso contract COVID-19 by touching a contaminated surface and then their\nface.&nbsp;It is most contagious when people are symptomatic, although spread\nmay be possible before symptoms appear.&nbsp;The virus can survive on surfaces\nup to 72 hours.&nbsp;Time from exposure to onset of symptoms is generally\nbetween two and fourteen days, with an average of five days.&nbsp;The standard\nmethod of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Diagnosis\">diagnosis<\/a>&nbsp;is by&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Reverse_transcription_polymerase_chain_reaction\">reverse transcription polymerase chain\nreaction<\/a>&nbsp;(rRT-PCR)\nfrom a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Nasopharyngeal_swab\">nasopharyngeal\nswab<\/a>.\nThe infection can also be diagnosed from a combination of symptoms,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Risk_factor\">risk factors<\/a>&nbsp;and a\nchest&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/CT_scan\">CT\nscan<\/a>&nbsp;showing\nfeatures of pneumonia.<\/p>\n\n\n\n<p>Recommended measures to prevent infection\ninclude frequent&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hand_washing\">hand washing<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Social_distancing\">social distancing<\/a>&nbsp;(maintaining\nphysical distance from others, especially from those with symptoms), covering\ncoughs and sneezes with a tissue or inner elbow, and keeping unwashed hands\naway from the face.&nbsp;The use of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Masks\">masks<\/a>&nbsp;is recommended\nfor those who suspect they have the virus and their\ncaregivers.&nbsp;Recommendations for mask use by the general public vary, with\nsome authorities recommending against their use, some recommending their use,\nand others requiring their use.&nbsp;Currently, there is no&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Vaccine\">vaccine<\/a>&nbsp;or\nspecific&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Antiviral_treatment\">antiviral\ntreatment<\/a>&nbsp;for\nCOVID-19. Management involves&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Palliative_care\">treatment of symptoms<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Supportive_care\">supportive care<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Isolation_(health_care)\">isolation<\/a>, and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Medical_research\">experimental measures<\/a>.<\/p>\n\n\n\n<p>The&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/World_Health_Organization\">World Health Organization<\/a>&nbsp;(WHO) declared\nthe 2019\u201320 coronavirus&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Outbreak\">outbreak<\/a>&nbsp;a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Public_Health_Emergency_of_International_Concern\">Public Health\nEmergency of International Concern<\/a>&nbsp;(PHEIC)&nbsp;on 30 January 2020, and\na&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pandemic\">pandemic<\/a>&nbsp;on 11 March\n2020.&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Local_transmission\">Local\ntransmission<\/a>&nbsp;of\nthe disease has been recorded in many countries across all six&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/WHO_regions\">WHO regions<\/a>.<\/p>\n\n\n\n<p><strong>SIGNS AND SYMPTOMS <\/strong><\/p>\n\n\n\n<p>Those infected with the virus may be&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Asymptomatic_carrier\">asymptomatic<\/a>&nbsp;or\ndevelop&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Influenza-like_illness\">flu-like symptoms,<\/a>&nbsp;including\nfever, cough, fatigue, and shortness of breath.&nbsp;Emergency symptoms include\ndifficulty breathing, persistent chest pain or pressure, confusion, difficulty\nwaking, and bluish face or lips; immediate medical attention is advised if\nthese symptoms are present.&nbsp;Less commonly,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Upper_respiratory\">upper respiratory<\/a>&nbsp;symptoms, such\nas&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Sneeze\">sneezing<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Rhinorrhoea\">runny nose<\/a>, or&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Sore_throat\">sore throat<\/a>&nbsp;may be seen.\nSymptoms such as&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Nausea\">nausea<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Vomiting\">vomiting<\/a>, and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Diarrhea\">diarrhea<\/a>&nbsp;have been\nobserved in varying percentages.&nbsp;Some cases in China initially presented\nonly with&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Chest_pain\">chest tightness<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Palpitations\">palpitations<\/a>.&nbsp;In March 2020\nthere were reports indicating that&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Anosmia\">loss of the sense of smell<\/a>&nbsp;(anosmia) may\nbe a common symptom among those who have mild disease, although not as common\nas initially reported.&nbsp;In some, the disease may progress to&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pneumonia\">pneumonia<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Multiple_organ_dysfunction_syndrome\">multi-organ failure<\/a>, and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Death\">death<\/a>. In those who develop severe symptoms, time from symptom\nonset to needing mechanical ventilation is typically eight days.<strong><\/strong><\/p>\n\n\n\n<p>As is common with infections, there is a delay between the\nmoment when a person is infected with the virus and the time when they develop\nsymptoms. This is called the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Incubation_period\">incubation\nperiod<\/a>. The incubation period for COVID-19\nis typically five to six days but may range from two to 14 days.&nbsp;97.5% of\npeople who develop symptoms will do so within 11.5 days of infection.<\/p>\n\n\n\n<p>Reports indicate that not all who are infected develop\nsymptoms, but their role in transmission is unknown.&nbsp;Preliminary evidence\nsuggests asymptomatic cases may contribute to the spread of the\ndisease.&nbsp;The proportion of infected people who do not display symptoms is\ncurrently unknown and being studied, with South Korea&#8217;s CDC reporting that 20%\nof all confirmed cases remained asymptomatic during their hospital stay.<\/p>\n\n\n\n<p><strong>TRANSMISSION<\/strong><\/p>\n\n\n\n\n\n<p><a href=\"https:\/\/en.wikipedia.org\/wiki\/File:Sneeze.JPG\"><\/a><sup><\/sup><\/p>\n\n\n\n<p>Respiratory droplets may also be produced\nduring breathing out, including when talking. Though the virus is not\ngenerally&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Airborne_disease\">airborne<\/a>,&nbsp;The National\nAcademy of Science has suggested that&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Bioaerosol\">bioaerosol<\/a>&nbsp;transmission\nmay be possible and air collectors positioned in the hallway outside of\npeople&#8217;s rooms yielded samples positive for viral RNA. The droplets can land in\nthe mouths or noses of people who are nearby or possibly be inhaled into the\nlungs.&nbsp;Some medical procedures such as intubation and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Cardiopulmonary_resuscitation\">cardiopulmonary resuscitation<\/a>&nbsp;(CPR) may cause\nrespiratory secretions to be aerosolised and thus result in airborne\nspread.&nbsp;It may also spread when one touches a contaminated surface, known\nas&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fomite\">fomite<\/a>&nbsp;transmission,\nand then touches their eyes, nose, or mouth.&nbsp;While there are concerns it\nmay spread by&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fecal%E2%80%93oral_route\">feces<\/a>, this risk is\nbelieved to be low. <\/p>\n\n\n\n<p>The virus is most contagious when people are\nsymptomatic; while spread may be possible before symptoms appear, this risk is\nlow.&nbsp;The&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/European_Centre_for_Disease_Prevention_and_Control\">European Centre for\nDisease Prevention and Control<\/a>&nbsp;(ECDC) states that while it is not\nentirely clear how easily the disease spreads, one person generally infects two\nto three others. <\/p>\n\n\n\n<p>The virus survives\nfor hours to days on surfaces.&nbsp;Specifically, the virus was found to be\ndetectable for one day on cardboard, for up to three days on plastic and\nstainless steel, and for up to four hours on copper.&nbsp;This, however, varies\nbased on the humidity and temperature.&nbsp;Surfaces may be decontaminated with\na number of solutions (within one minute of exposure to the disinfectant for a\nstainless steel surface), including 62\u201371%&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Ethanol\">ethanol<\/a>, 50\u2013100%&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Isopropanol\">isopropanol<\/a>, 0.1%&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Sodium_hypochlorite\">sodium\nhypochlorite<\/a>,\n0.5%&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hydrogen_peroxide\">hydrogen\nperoxide<\/a>,\nand 0.2\u20137.5%&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Povidone-iodine\">povidone-iodine<\/a>. Other solutions,\nsuch as&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Benzalkonium_chloride\">benzalkonium\nchloride<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Chlorhexidine\">chrohexidine\ngluconate<\/a>,\nare less effective.<\/p>\n\n\n\n<p><strong>VIROLOGY<\/strong><\/p>\n\n\n\n\n\n<p><a href=\"https:\/\/en.wikipedia.org\/wiki\/File:Coronavirus_virion_structure.svg\"><\/a><\/p>\n\n\n\n<p>Severe acute respiratory syndrome coronavirus\n2 (SARS-CoV-2) is a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Novel_virus\">novel<\/a>&nbsp;severe acute\nrespiratory syndrome coronavirus, first isolated from three people with\npneumonia connected to the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Disease_cluster\">cluster<\/a>&nbsp;of acute\nrespiratory illness cases in Wuhan.&nbsp;All features of the novel SARS-CoV-2\nvirus occur in related coronaviruses in nature.<\/p>\n\n\n\n<p>Outside the human body, the virus is killed\nby household&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Soap\">soap<\/a>, which bursts its\nprotective bubble.<\/p>\n\n\n\n<p>SARS-CoV-2 is closely related to the original\nSARS-CoV.&nbsp;It is thought to have a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Zoonosis\">zoonotic<\/a>&nbsp;origin. Genetic\nanalysis has revealed that the coronavirus genetically clusters with the\ngenus&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Betacoronavirus\"><em>Betacoronavirus<\/em><\/a>, in subgenus&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Severe_acute_respiratory_syndrome-related_coronavirus\"><em>Sarbecovirus<\/em><\/a>&nbsp;(lineage B)\ntogether with two bat-derived strains. It is 96% identical at the whole&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Genome\">genome<\/a>&nbsp;level to other\nbat coronavirus samples (BatCov RaTG13).&nbsp;In February 2020, Chinese\nresearchers found that there is only one&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Amino_acid\">amino acid<\/a>&nbsp;difference in\ncertain parts of the genome sequences between the viruses from&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pangolins\">pangolins<\/a>&nbsp;and those from\nhumans, however, whole-genome comparison to date found at most 92% of genetic\nmaterial shared between pangolin coronavirus and SARS-CoV-2, which is\ninsufficient to prove pangolins to be the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Intermediate_host\">intermediate host<\/a>.<\/p>\n\n\n\n<p>The&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/World_Health_Organization\">WHO<\/a>&nbsp;has published several testing protocols for the\ndisease.&nbsp;The standard method of testing is real-time&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Reverse_transcription_polymerase_chain_reaction\">reverse transcription polymerase chain\nreaction<\/a>&nbsp;(rRT-PCR).&nbsp;The\ntest is typically done on respiratory samples obtained by a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Nasopharyngeal_swab\">nasopharyngeal\nswab<\/a>,\nhowever a nasal swab or&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Sputum\">sputum<\/a>&nbsp;sample may also\nbe used.&nbsp;Results are generally available within a few hours to two\ndays.&nbsp;Blood tests can be used, but these require two blood samples taken\ntwo weeks apart and the results have little immediate value. Chinese scientists\nwere able to isolate a strain of the coronavirus and publish the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Nucleic_acid_sequence\">genetic\nsequence<\/a>&nbsp;so\nlaboratories across the world could independently develop&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Polymerase_chain_reaction\">polymerase chain reaction<\/a>&nbsp;(PCR) tests to\ndetect infection by the virus. As of 19 March 2020.&nbsp;there were no antibody\ntests though efforts to develop them are ongoing.&nbsp;The FDA approved the\nfirst&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Point-of-care_testing\">point-of-care\ntest<\/a>&nbsp;on\n21 March 2020 for use at the end of that month. <\/p>\n\n\n\n<p>Diagnostic guidelines released by Zhongnan\nHospital of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Wuhan_University\">Wuhan\nUniversity<\/a>&nbsp;suggested\nmethods for detecting infections based upon clinical features and\nepidemiological risk. This diffuse DAD is\nresponsible of the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Acute_respiratory_distress_syndrome\">acute respiratory distress syndrome<\/a>&nbsp;(ARDS) and\nsevere&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hypoxemia\">hypoxemia<\/a>&nbsp;observed in\nthis disease.<\/li><\/ul><ul><li>healing\npneumonia:&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Healing\">organization<\/a>&nbsp;of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Exudate\">exudates<\/a>&nbsp;in&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pulmonary_alveolus\">alveolar\ncavities<\/a>,\nand&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pulmonary_fibrosis\">pulmonary\ninterstitial fibrosis<\/a><\/li><\/ul><ul><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Plasma_cell\">plasmocytosis<\/a>&nbsp;in&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Bronchoalveolar_lavage\">BAL<\/a><\/li><\/ul><\/li><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Blood\">Blood<\/a>:&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Disseminated_intravascular_coagulation\">disseminated intravascular coagulation<\/a>&nbsp;(DIC)&nbsp;;\nleukoerythroblastic reaction<\/li><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Liver\">Liver<\/a>: microvesicular&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Steatosis\">steatosis<\/a><\/li><\/ul>\n\n\n\n<p><strong>PREVENTIONS <\/strong><\/p>\n\n\n\n\n\n<p><a href=\"https:\/\/en.wikipedia.org\/wiki\/File:Covid-19-curves-graphic2-stopthespread-v3.gif\"><\/a><\/p>\n\n\n\n<p>Preventive measures to reduce the chances of\ninfection include staying at home, avoiding crowded places, washing hands with\nsoap and water often and for at least 20 seconds, practicing good\nrespiratory&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hygiene\">hygiene<\/a>&nbsp;and avoiding\ntouching the eyes, nose, or mouth with unwashed hands. The&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Centers_for_Disease_Control_and_Prevention\">CDC<\/a>&nbsp;recommends covering the mouth and nose\nwith a tissue when coughing or sneezing and recommends using the inside of the\nelbow if no tissue is available. They also recommend proper hand hygiene after\nany cough or sneeze. <a href=\"https:\/\/en.wikipedia.org\/wiki\/Social_distancing\">Social distancing<\/a>&nbsp;strategies aim\nto reduce contact of infected persons with large groups by closing schools and\nworkplaces, restricting travel, and canceling mass gatherings.&nbsp;Social\ndistancing also includes that people stay at least six feet apart (1.83\nmeters). <\/p>\n\n\n\n<p>Because a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/COVID-19_vaccine\">vaccine against\nSARS-CoV-2<\/a>&nbsp;is\nnot expected to become available until 2021 at the earliest, a key part of\nmanaging the COVID-19 pandemic is trying to decrease the epidemic peak, known\nas &#8220;flattening the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Epidemic_curve\">curve<\/a>&#8220;, through\nvarious measures seeking to reduce the rate of new infections.&nbsp;Slowing the\ninfection rate helps decrease the risk of health services being overwhelmed,\nallowing for better treatment of current cases, and delaying additional cases\nuntil&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Therapy\">therapeutics<\/a>&nbsp;or a vaccine\nbecome available. <\/p>\n\n\n\n<p>According to the WHO, the use of masks is\nrecommended only if a person is coughing or sneezing or when one is taking care\nof someone with a suspected infection.&nbsp;Some countries also recommend\nhealthy individuals to wear face masks, including China,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hong_Kong\">Hong Kong<\/a>, <a href=\"https:\/\/en.wikipedia.org\/wiki\/Thailand\">Thailand<\/a>,&nbsp;Czech\nRepublic,&nbsp;and Austria.&nbsp;In order to meet the need for masks, the WHO\nestimates that global production will need to increase by 40%. Hoarding and\nspeculation have worsened the problem, with the price of masks increasing\nsixfold,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/N95_masks\">N95\nrespirators<\/a>&nbsp;tripled,\nand gowns doubled.&nbsp;Some health experts consider wearing non-medical grade\nmasks and other face coverings like scarves or bandanas a good way to prevent\npeople from touching their mouths and noses, even if non-medical coverings\nwould not protect against a direct sneeze or cough from an infected person. <\/p>\n\n\n\n<p>Those diagnosed with COVID-19 or who believe\nthey may be infected are advised by the CDC to stay home except to get medical\ncare, call ahead before visiting a healthcare provider, wear a face mask before\nentering the healthcare provider&#8217;s office and when in any room or vehicle with\nanother person, cover coughs and sneezes with a tissue, regularly wash hands\nwith soap and water, and avoid sharing personal household items.&nbsp;The CDC\nalso recommends that individuals wash hands often with soap and water for at\nleast 20 seconds, especially after going to the toilet or when hands are\nvisibly dirty, before eating and after blowing one&#8217;s nose, coughing, or\nsneezing. It further recommends using an alcohol-based&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hand_sanitiser\">hand sanitiser<\/a>&nbsp;with at least\n60% alcohol, but only when soap and water are not readily available. <\/p>\n\n\n\n<p>For areas where commercial hand sanitisers\nare not readily available,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/World_Health_Organization\">WHO<\/a>&nbsp;provides two formulations for local production. In\nthese formulations, the antimicrobial activity arises from&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Ethanol\">ethanol<\/a>&nbsp;or&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Isopropanol\">isopropanol<\/a>.&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hydrogen_peroxide\">Hydrogen peroxide<\/a>&nbsp;is used to help\neliminate bacterial spores in the alcohol; it is &#8220;not an active substance\nfor hand&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Antisepsis\">antisepsis<\/a>&#8220;.&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Glycerol\">Glycerol<\/a>&nbsp;is added as\na&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Humectant\">humectant<\/a>. <\/p>\n\n\n\n<p>Prevention efforts\nare multiplicative, with effects far beyond that of a single spread. Each\navoided case leads to more avoided cases down the line, which in turn can stop\nthe outbreak in its tracks.<\/p>\n\n\n\n<p><strong>HISTORY<\/strong><\/p>\n\n\n\n<p>The virus is thought to be natural and have\nan&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Zoonosis\">animal origin<\/a>,&nbsp;through&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Spillover_infection\">spillover\ninfection<\/a>.&nbsp;The\norigin is unknown but by December 2019 the spread of infection was almost\nentirely driven by human-to-human transmission. The earliest reported infection\nhas been unofficially reported to have occurred on 17 November 2019 in&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/2019%E2%80%9320_coronavirus_pandemic_in_Hubei\">Wuhan, China<\/a>.&nbsp;A study of the\nfirst 41 cases of confirmed COVID-19, published in January 2020 in&nbsp;<em>The\nLancet<\/em>, revealed the earliest date of onset of symptoms as 1 December\n2019.&nbsp;Official publications from the WHO reported the earliest onset of\nsymptoms as 8 December 2019. <\/p>\n\n\n\n<p><strong>EPIDIMIOLOGY<\/strong><\/p>\n\n\n\n<p>Several measures are commonly used to\nquantify mortality.&nbsp;These numbers vary by region and over time, and are\ninfluenced by the volume of testing, healthcare system quality, treatment\noptions, time since initial outbreak, and population characteristics such as age,\nsex, and overall health.&nbsp;In late 2019, WHO assigned the emergency&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/ICD-10\">ICD-10<\/a>&nbsp;disease\ncodes&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/ICD-10_Chapter_XXII:_Codes_for_special_purposes\">U07.1<\/a>&nbsp;for deaths from\nlab-confirmed SARS-CoV-2 infection and U07.2 for deaths from clinically or\nepidemiologically diagnosed COVID-19 without lab-confirmed SARS-CoV-2\ninfection.<\/p>\n\n\n\n<p>The death-to-case ratio reflects the number\nof deaths divided by the number of diagnosed cases within a given time\ninterval. Based on Johns Hopkins University statistics, the global\ndeath-to-case ratio is&nbsp;5.2% (53,069\/1,016,128) as of 3 April\n2020.&nbsp;The number varies by region.<\/p>\n\n\n\n<p>Other measures include the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Case_fatality_rate\">case\nfatality rate<\/a>&nbsp;(CFR),\nwhich reflects the percent of diagnosed individuals who die from a disease, and\nthe infection fatality rate (IFR), which reflects the percent of infected\nindividuals (diagnosed and undiagnosed) who die from a disease. These\nstatistics are not time bound and follow a specific population from infection\nthrough case resolution. A number of academics have attempted to calculate\nthese numbers for specific populations.<\/p>\n\n\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/File:Total-cases-covid-19-who.png\"><\/a><\/li><\/ul>\n\n\n\n<p>Total confirmed cases\nover time<\/p>\n\n\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/File:Total-deaths-covid-19-who_(1).png\"><\/a><\/li><\/ul>\n\n\n\n<p>Total deaths over\ntime<\/p>\n\n\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/File:Total-confirmed-cases-of-covid-19-per-million-people.png\"><\/a><\/li><\/ul>\n\n\n\n<p>Total confirmed cases\nof COVID-19 per million people, 20 March 2020<\/p>\n\n\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/File:Total-covid-deaths-per-million.png\"><\/a><\/li><\/ul>\n\n\n\n<p>Total confirmed\ndeaths due to COVID-19 per million people, 24 March 2020<\/p>\n\n\n\n<p><strong>RESEARCH <\/strong><\/p>\n\n\n\n<p>Because of its key role in the transmission\nand progression of SARS-CoV-2,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Angiotensin-converting_enzyme_2\">ACE2<\/a>&nbsp;has been the focus of a significant\nproportion of research and various&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Therapy\">therapeutic<\/a>&nbsp;approaches have\nbeen suggested.&nbsp;Personal hygiene, and a healthy lifestyle and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Healthy_diet\">diet<\/a>&nbsp;have been recommended\nto improve immunity. <\/p>\n\n\n\n<h4 class=\"wp-block-heading\">v Vaccine<\/h4>\n\n\n\n<p>There is no available vaccine, but research\ninto developing a vaccine has been undertaken by various agencies. Previous\nwork on&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/SARS-CoV\">SARS-CoV<\/a>&nbsp;is being utilised\nbecause SARS-CoV-2 and SARS-CoV both use the ACE2 receptor to enter human\ncells.&nbsp;There are three vaccination strategies being investigated. First,\nresearchers aim to build a whole virus vaccine. The use of such a virus, be\nit&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Inactivated_vaccine\">inactive<\/a>&nbsp;or dead, aims\nto elicit a prompt&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Immune_response\">immune response<\/a>&nbsp;of the human\nbody to a new infection with COVID-19. A second strategy, subunit vaccines,\naims to create a vaccine that sensitises the immune system to certain subunits\nof the virus. In the case of SARS-CoV-2, such research focuses on the S-spike\nprotein that helps the virus intrude the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/ACE2_enzyme\">ACE2 enzyme<\/a>&nbsp;receptor. A\nthird strategy is that of the nucleic acid vaccines (<a href=\"https:\/\/en.wikipedia.org\/wiki\/DNA_vaccination\">DNA<\/a>&nbsp;or&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/RNA_vaccines\">RNA vaccines<\/a>, a novel technique\nfor creating a vaccination). Experimental vaccines from any of these strategies\nwould have to be tested for safety and efficacy. <\/p>\n\n\n\n<p>On 16 March 2020, the first clinical trial of\na vaccine started with four volunteers in&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Seattle\">Seattle<\/a>. The vaccine\ncontains a harmless genetic code copied from the virus that causes the disease.\n<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">v Antivirals<\/h4>\n\n\n\n<p>Several existing&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Antiviral_drug\">antiviral medications<\/a>&nbsp;are being\nevaluated for treatment of COVID-19 and some have moved into clinical\ntrials.&nbsp;In March 2020, WHO launched a multi-country trial involving 10\ncountries called &#8220;Solidarity&#8221; in response to COVID-19 pandemic.&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Remdesivir\">Remdesivir<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Chloroquine\">chloroquine<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hydroxychloroquine\">hydroxychloroquine<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Lopinavir\/ritonavir\">lopinavir\/ritonavir<\/a>, and\nlopinavir\/ritonavir combined with&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Interferon-beta\">interferon beta<\/a>&nbsp;are the\nexperimental treatments currently being researched under Solidarity Trial. <\/p>\n\n\n\n<p>There is tentative evidence for remdesivir as\nof March 2020.&nbsp;Remdesivir inhibits SARS-CoV-2&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/In_vitro\"><em>in vitro<\/em><\/a>.&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Phase_3_clinical_trial\">Phase\n3 clinical trials<\/a>&nbsp;are\nbeing conducted in the U.S., China, and Italy. <\/p>\n\n\n\n<p>Chloroquine, previously used to treat&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Malaria\">malaria<\/a>, was studied in\nChina in February 2020, with positive preliminary results.&nbsp;However, there\nare calls for peer review of the research.&nbsp;The Guangdong Provincial Department\nof Science and Technology and the Guangdong Provincial Health and Health\nCommission issued a report stating that chloroquine phosphate &#8220;improves\nthe success rate of treatment and shortens the length of person&#8217;s hospital\nstay&#8221; and recommended it for people diagnosed with mild, moderate and\nsevere cases of novel coronavirus pneumonia. <\/p>\n\n\n\n<p>On 17 March, the Italian Pharmaceutical\nAgency included chloroquine and hydroxychloroquine in the list of drugs with\npositive preliminary results for treatment of COVID-19.&nbsp;Korean and Chinese\nHealth Authorities recommend the use of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Chloroquine\">chloroquine<\/a>.&nbsp;However, the\nWuhan Institute of Virology, while recommending a daily dose of one gram, notes\nthat twice that dose is highly dangerous and could be lethal. On 28 March 2020,\nthe&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Food_and_Drug_Administration\">FDA<\/a>&nbsp;issued an&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Emergency_use_authorization\">emergency use authorization<\/a>&nbsp;for\nhydroxychloroquine and chloroquine at the discretion of physicians treating\npeople with COVID-19. <\/p>\n\n\n\n<p>The Chinese 7th edition guidelines also\ninclude&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Interferon\">interferon<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Ribavirin\">ribavirin<\/a>, or&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Umifenovir\">umifenovir<\/a>&nbsp;for use against\nCOVID-19. <\/p>\n\n\n\n<p>In 2020, a trial found that\nlopinavir\/ritonavir was ineffective in the treatment of severe illness. <a href=\"https:\/\/en.wikipedia.org\/wiki\/Nitazoxanide\">Nitazoxanide<\/a>&nbsp;has been\nrecommended for further&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/In_vivo\"><em>in vivo<\/em><\/a>&nbsp;study after\ndemonstrating low concentration inhibition of SARS-CoV-2. <\/p>\n\n\n\n<p>Studies have demonstrated that initial spike\nprotein priming by transmembrane protease serine&nbsp;2\n(<a href=\"https:\/\/en.wikipedia.org\/wiki\/TMPRSS2\">TMPRSS2<\/a>) is essential for\nentry of SARS-CoV-2 via interaction with the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/ACE2\">ACE2<\/a>&nbsp;receptor.&nbsp;These findings\nsuggest that the TMPRSS2 inhibitor&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Camostat\">camostat<\/a>&nbsp;approved for\nuse in Japan for inhibiting fibrosis in liver and kidney disease might\nconstitute an effective off-label treatment. <\/p>\n\n\n\n<p>In February 2020,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Favipiravir\">favipiravir<\/a>&nbsp;was being\nstudied in China for experimental treatment of the emergent COVID-19 disease. <\/p>\n\n\n\n<h4 class=\"wp-block-heading\">v Anti-cytokine\nstorm<\/h4>\n\n\n\n<p><a href=\"https:\/\/en.wikipedia.org\/wiki\/Cytokine_release_syndrome\">Cytokine storm<\/a>, a life-threatening medical condition, can\nbe a complication in the later stages of severe COVID-19.
